I do not have any information regarding living wills in other countries. So if you have further information or can point to a link for forms please post.

Folks, some things to think about...
I have noticed many people forget their passwords. With so many it's hard to keep track. But with having thyroid disease comes the brain fog and many other things.
What IF you were to become ill enough that you for instance ended up in the hospital? Or had an accident? Or worse...Would your spouse, parents, or other person know how to pay your online bills and utilities? Do you have a disk made with instructions in case of emergencies? If not, perhaps you should.
Do you have a family health notebook? Do you know what vaccinations you've had? What about your kids? Do you keep copies of your labwork? Do you keep a list of dr's with phone numbers readily available in case of an emergency? Does your family know what prescription drugs and supplements you take? Creating a notebook can be very useful. Emergencies do happen and often people panic and cannot remember things.
Do you have an advanced directive? And a will prepared? Lots of people know what they want and what they don't want. But didn't put it on paper and this creates problems for families. If you have neither, have thought about it, or put it off...Maybe it's time to think about these things more seriously and get them taken care of.
How prepared would you (your spouse, significant other, or parent) be in case of an emergency?

I have both an advance directive and a will prepared. It is SO very important. One reason I stress an advance directive is you cannot depend on a loved one to make the right choice for you because it simply is the hardest decision for a family to make.
When I was in the hospital I was asked a lot of things and this was one of them. Even though I knew what I wanted I didn't have it on a signed piece of paper. Although I tried talking to my dr about it I was simply reassured with a smile that everything would be ok. Meanwhile...across the hall. Code Blue.
